Wanneer patiënten die Tarceva - erlotinib of Iressa - gefitinib krijgen om hun longkanker of andere vormen van kanker bv. alvleesklierkanker - vormen van darmkanker) te bestrijden en daarvan veel last hebben van bijwerkingen in de vorm van huidirritaties, jeuk en acne-achitige plekken dan blijkt dit een onafhankelijk voorspellende factor te zijn dat de behandeling juist goed aanslaat. Vooral ook bij patiënten waar de EGFR status niet van bekend is. Dit blijkt uit een grote meta analyse van 24 gerandomiseeerde studies. Alleen al de objectieve repsons gaf een verschil te zien van 42% voor patiënten die dergelijke bijwerkingen vertoonden tegenover 7% bij wie geen of nauwelijks bijwerkingen werden gezien. De progressievrije tijd en overall overlevingscijfers waren ook significant beter voor de patiënten die wel bijwerkingen in de vorm van huidirritaties en acne lieten zien:  overleving (HR: 0.30; p<0.00001) en progressie vrij (HR: 0.50; p<0.00001). Er werd geen verschil gezien tussen Tarceva - erlotinib en Iressa - Gefitinib. De resultaten gelden voor beide middelen.

Oncologen wordt aangeraden bij optredende bijwerkingen, die door de patiënten als vervelend worden ervaren omdat het ook vaak in het gezicht voorkomt, te benadrukken dat dit juist een teken is van aanslaan van de behandeling. Later verdwijnen deze bijwerkingen ook weer.

Abstract 

Background

Dermatological toxicity, usually in the form of acneiform rash, is frequently observed in non-small-cell lung cancer (NSCLC) patients treated with anti-EGF receptor (EGFR) tyrosine kinase inhibitors (TKIs). The objective of this review was to assess the predictive value of skin rash for outcome in patients with NSCLC treated with erlotinib and gefitinib.

Methods

We searched PubMed for articles reporting a correlation of skin rash with survival, progression and response rate. In total, 349 prospective or retrospective studies presenting data regarding patient outcome and skin toxicity were screened. Hazard ratios (HRs) with 95% confidence intervals for progression and survival and risk ratios (RRs) for response rate were obtained from these publications and pooled in a meta-analysis.

Results

This meta-analysis included 24 publications (17 prospective trials and 7 retrospective case series). Skin rash was found to be an independent predictive factor for survival (HR: 0.30; p<0.00001) and progression (HR: 0.50; p<0.00001). In addition, patients who developed grade 2–4 rash were more likely to respond to treatment respect to patients with no rash (42% vs. 7%). The result for survival meta-analysis appears to be similar for gefitinib and erlotinib.

Conclusion

    Hallo

    Mijn moeder slikt sinds anderhalf jaar Iressa
    Ze heeft de laatste tijd veel last van haar maag, onderzoek gehad en deze bleek behoorlijk geirriteerd te zijn. Omeprazol blijkbaar fout voorgeschreven want volgens de longarts zou de Iressa dan zijn werk niet meer doen. Inmiddels verliest mijn moeder haar eetlust en wordt misselijk bij het koken. Onderzoek naar de gal gehad er blijken galstenen te zitten, maar zitten niet op die plek die deze symptonen kunnen veroorzaken ze heeft ook aanvallen van duizeligheid af en toe. Wat moeten we nu ze heeft ook last van diaree. Ze heeft longkanker maar soms praat de arts ook over dood weefsel. Ze heeft atrose in haar nek volgens de neuroloog, maar de longarts zegt weer wat nou atrose dat zijn verse tumoortjes. Ik zou voor mijn moeder graag een second opinion aanvragen !
